Download Your State's Advance Directive

Advance directives are legal documents that help others understand your desires if you become seriously ill or incapacitated. Advance directives typically include a living will form to document your end-of-life care preferences, a healthcare proxy to identify who should make your medical decisions, and a durable power of attorney to identify who will make your legal and financial decisions.

These advance directive forms are provided by the National Hospice and Palliative Care Organization. Before completing a binding legal document, we strongly recommend a free consultation with a Care Guide and working with an attorney experienced in drafting advance healthcare directives.
